At Cantellops & Family Pediatric Clinic, your child will be seen by one of our two board-certified primary care physicians:

  • Dr. José M. Cantellops Montalvo – General Pediatrician

  • Dr. Joao M. Cantellops Cueli – General Pediatrician

 

Both participate in the Plan Vital network with insurers such as Triple-S, First Medical, Menonitas and MMM, and are affiliated with the IPA 302 and IPA 300.

 

If your child is insured with Plan Vital, here's everything you need to know:

This guide was made for you, mom or dad, who has a million things on your mind. We'll explain it clearly and simply.

How to choose one of our pediatricians with the Plan Vital?

1️⃣ If you are new to Plan Vital

Sometimes, a doctor is assigned to you automatically by the plan. But you can change it from the start.

What to do?
Call your insurance company and request to be assigned to:

  • Dr. José M. Cantellops Montalvo (NPI: 1073578068)

  • Dr. Joao M. Cantellops Cueli (NPI: 1871147967)

Give them their full name with their NPI, let them know that they are affiliated with the IPA 302 and IPA 300, and confirm the assignment date.

 

2️⃣ If you are already registered with Plan Vital but want to change your primary care physician

You can change doctors once every 6 months, or sooner if there is a medical reason.

What to do?

  1. Call your insurer's customer service number (located in the back of your medical card).

  2. Ask to make the change to one of our pediatrician.

  3. Check:

    • That they spelled the name correctly

    • The date on which the change will take effect

Your rights as a patient under Plan Vital

As a parent with a Vital Plan, you and your children have these rights:

  • 👨‍⚕️ Choosing and changing your primary care physician (PCP).

  • 🗓️ Receive medical care without undue delay.

  • 💉 Access to preventative services such as vaccines, developmental assessment, etc.

  • 🧑‍⚕️ Seek a second medical opinion if you are not satisfied.

  • 🏥 Accessing emergency services without prior authorization.

  • 💊 Receive prescription medications covered by the plan.

  • 📂 Request a copy of your child's medical record.

  • ❌ Refusing treatments if you do not agree.

  • 💬 Submit complaints and appeals if you feel something is wrong.

  • 👩‍⚖️ To be treated without discrimination based on race, gender, condition, etc.
